About Us

We are RN owned and operated Registered Nurse Care Coordinators and patient advocates. We assist patients navigate the complexities of the healthcare system. This entails managing all aspects of our client's healthcare needs from medication management/administration, making and attending medical appointments with them, providing hospital stay guidance/support.

Our RN Care Coordinators provide excellent medical advocacy to support the medical needs when family members or loved ones are unable to be present, whether it's due to a busy schedule or living out of town. We closely collaborate with family members and all service providers. RN Care Managers embrace a holistic approach to managing quality of life by caring for every one of our clients as a whole. We continue to monitor the needs of the aging individual; we schedule regular visits to re-assess the health and well-being of our clients. We understand how overwhelming it can be to care for loved ones, and we're here to take those important steps with you, guiding you every step of the way. Our goal is to help prevent hospitalizations and provide you with a higher quality of life.

Agape clients have their personal RN Care Manager who is available 24/7 in the event of a medical health crisis situation.

We want to help stop the revolving door to a maze of doctors and multiple hospital admissions.
