Family Mentoring Program Case Manager
Purpose:
This position is responsible for maintaining, training, supporting, and developing our churches, clients, and family mentors. This position fulfills the purpose of carrying out the mission statement of Hill Country Daily Bread Ministries (HCDBM) by implementing ministry foundations to help both the church’s family mentoring ministry and clients to succeed in meeting their spiritual, physical, and emotional goals.
Position Description:
The Family Mentoring Case Manager will be held accountable for each of these areas in their day-to-day office time. These factors will include but are not limited to: Case Management of Churches and Clients, Mentor Onboarding processes, and HCDBM office collaboration.
* Reports To: Family Mentoring Resource Program Case Manager Supervisor
* Salary: Salary dependent on experience and education
* Schedule: Monday-Friday, 9:00am-5:00pm Occasional evening and weekends are required for this position
Qualifications:
* Minimum Education: high school diploma or equivalent. Preferred associate or bachelor's degree in Social Work, Psychology, Human Services, or related field (or equivalent experience)
* Minimum of 2 years of case management, social work, or human services experience required
* Ability to work with diverse populations in a compassionate, nonjudgmental manner
* Comfortable offering spiritual support in line with Hill Country Daily Bread Ministries' Christian mission
* Bilingual (English/Spanish) preferred but not required
* Proficiency in Microsoft Office 365 program and comfortable learning case management software
* Strong customer service and interpersonal communication skills (verbal and written)
* Organized and administrative with strong attention to detail
* Excellent problem-solving skills
* Proven ability to work efficiently both independently as well as part of a team
* Ability and willingness to drive and travel to church partner sites for client intakes and mentor matches.
* Must pass background check and have reliable transportation
Responsibilities:
* Provide ongoing case management, including follow-up, accountability, and encouragement to an active caseload of mentors and their client families following HCDBM Case Management Guidelines.
* Conduct client intake interviews and needs assessments
* Connect clients with internal programs and community resources
* Collaborate with the Partner Support Specialist in facilitating matches between clients and mentors/mentoring churches.
* Ensure reporting & data management for clients and churches accurately and in a timely manner
* Develop and maintain supportive relationships with assigned churches and their respective teams
* Gather and report stories and testimonials from clients to Case Manager Supervisor
* Maintain compliance with department structure, processes, and ministry goals.
* Work in conjunction with department team members to build an excellent ministry through obedience to God’s plan.
